Your first and second preferred appointment dates
The student's first and last name
The student's shirt, pants, underwear, and shoe sizes (see the attached CAP Size Guide). If shoes are not needed, please indicate "No shoes."
If you have other JCPS students living in your household, they may also receive clothing or school uniforms (if required by their school). Please include each student's name and clothing sizes.
Appointments
CAP scheduling opens Thursday, July 16. Ms. Davis will begin scheduling appointments for Lassiter Middle School families at that time.
Appointments are scheduled on a first-come, first-served basis. Ms. Davis will do her best to schedule your appointment on one of your preferred dates between 10:00 a.m. and 1:00 p.m. Please note that specific appointment times cannot be guaranteed. Once the appointment is made, Ms. Davis will text or email a confirmation letter with the date, time, and address of CAP.
Remember to sign your 6th grade student for Camp Lassiter! More information is found in the attached link and image!

You can still register your student for Summer Learning at Lassiter! We are here everyday from 8:00-1:30. Students get a breakfast and lunch! We are having fun everyday! Next week we will have cooking/baking day on Monday and a cookout and smore's day on Thursday!

Today, June 8, 2026, is the final day to request bus transportation for the first day of school. Every JCPS family must complete the Transportation Verification (TV) form to let us know your plans.
Eligible bus riders who fill out the form by the end of the day today will be notified of their bus stop location in July via the JCPS Bus Teller. Don't delay—get your request in now!
