Parents/Guardians:
Thank you for your child’s application to attend Butler Traditional High School. We want to be able to consider you application as efficiently as possible.
Please send the “Choices Application” ONLY (yellow copy) to the office below by January 8, 2012:

Please send the following documents to Butler as soon as possible:
- A copy of student’s transcript
- A copy of student’s most recent Achievement Test Score
- A copy of student’s most Recent Report Card
- The enclosed Student Profile Survey
- Hand written answers by the applicant to the essay questions on the back of the Student Profile Survey, not to exceed one page.

**Students who apply for the traditional program must submit an application to the Office of Options and Magnets. To ensure success in the traditional curriculum, a review is made of each applicant’s scores, previous and current grades, and attendance record before the student is admitted. In addition, to enter Grade 9, all students must pass English, Mathematics, Science, and Social Studies in Grade 8.
