Transcripts

A student transcript for any Jefferson County Public School can be obtained by picking it up in person between 7:30 a.m. and 4:15 p.m. at 4309 Bishop Lane or through a mail-in request.  There is a $5.00 fee per high school transcript/written verification of graduation or $8.00 per fee elementary through high school transcript payable by cash/check or money order.  Most colleges and employers ONLY require a high school transcript.

In order for us to comply with the Family Educational Rights and Privacy Act regarding Disclosure of Education Records, please print and complete the online transcript form OR follow instructions from the appropriate category below:

If you are the INDIVIDUAL requesting your student transcript, provide a written request with the following:

  1. Your signature on the request for records
  2. Copy of a photo ID with signature to verify your identity
  3. The name under which you attended school (such as maiden name)
  4. Your date of birth
  5. The name of the school from which you graduated (school name on diploma)
  6. The year of graduation
  7. The address to which you want the transcript mailed
  8. The type of transcript you are requesting – high school or elementary through high school

If you are a BUSINESS OR GOVERNMENT AGENCY, provide a written request with the following:

  1. A signed release by the student
  2. Copy of the student’s photo ID with signature to verify their identity
  3. The name under which they attended school (such as maiden name)
  4. The student’s date of birth
  5. The name of the school from which they graduated (school name on diploma)
  6. The year of graduation
  7. The address to which you want the transcript mailed
  8. The type of transcript you are requesting – high school or elementary through high school

If you wish to have someone pick up the transcript for you, such as a parent or guardian, follow the requirements for a business or government agency. (Once a student turns 18, the rights of the parent is transferred to the student.)


If you are an EDUCATIONAL FACILITY, provide a written request with the following student information:

  1. A signed release by the student
  2. The name under which they attended school (such as maiden name)
  3. The student’s date of birth
  4. The name of the school from which they graduated (school name on diploma)
  5. The year of graduation
  6. The address to which you want the transcript mailed
  7. The type of transcript you are requesting – high school or elementary through high

Please mail the student information (along with the check/money order made out to Jefferson County Public Schools) to:

Unfortunately, time does not permit us to verify receipt of each request.  For verification of receipt, please send request through certified mail.  ALL requests will be processed within 48 hours of receipt unless information is incomplete or payment has not been received.

GED information is archived by the state.
