PD Forms
The procedure for receiving professional development credit for Dialogue is:
- All District Dialogue Team members and visiting principal(s) should sign the PD Attendance Roster
- Chairperson should make and keep a copy of this roster for your records. Within a week, send the original roster to: Deven Vaught, JCPS Planning Unit, VanHoose Education Center. Phone: 485-6254. Email: deven.vaught@jefferson.kyschools.us
- All dialogue team members should submit one PD evaluation form electronically through PDCentral after all school dialogues are completed.

- Elementary - October 3, 2006
- Middle School - September 20, 2006
- High School - September 29, 2006
Final Reports:
Dialogue Team Chairs should submit your final reports to Deven Vaught
