Middle and High School Applications
All Optional and Magnet Program applications must be received in the JCPS Optional, Magnet, and Advance Programs Office, Lam Building, 4309 Bishop Lane, Louisville, Kentucky 40218, (fax 485-3936), no later than January 9, 2009, to be considered for middle and high school placement for the 2009-10 school year.

Elementary School Applications
The elementary school application period will be held from February 2 through March 2, 2009. All elementary applications can be obtained from the student's resides elementary school. The resides school information can be accessed by calling the JCPS Demographics Department at 485-3050. Please return the yellow and white copies of the registration form to the child's resides elementary school to register for the cluster choices. To register for all Optional and Magnet choices, please forward the pink copy of the application to the Optional, Magnet, and Advance Program Office, Lam Building, 4309 Bishop Lane, Louisville, Kentucky 40218, (fax 485-3936 or 485-7155), no later than March 2, 2009, to be considered for elementary school placement for the 2007-08 school year.
